# After the last dose: what studies show

*Guide · 2026-10-22 · https://undiet.app/guides/after-the-last-dose*

The one large trial that followed people for a year after stopping semaglutide, what it reported about regain, what that does and does not mean for you, and how tracking, private photos and a community keep the record going when the therapy ends.

## The trial that kept watching

Most medication trials end on the last dose. STEP 1, the 68 week trial of weekly semaglutide 2.4 mg in 1961 adults, invited people back for a further year with no treatment and no counselling.

The extension analysis included 327 people, 228 from the semaglutide group and 99 from placebo, followed to week 120. The authors called it exploratory: no statistical tests, and a smaller group than the trial itself.

## What it reported

At week 68 the semaglutide group in the extension was 17.3% below its starting weight, and the placebo group 2.0% below. The therapy stopped there.

By week 120 the semaglutide group had regained 11.6 percentage points, leaving it 5.6% below the start. The placebo group regained 1.9 points and ended 0.1% below the start.

The authors summed it up as two thirds of the lost weight regained within a year, with the regain slowing towards the end of the year. At week 120, 48.2% of the semaglutide group was still at least 5% below where it began, against 22.6% of the placebo group.

## What that does and does not mean

It means the months after the last dose are not a footnote. In the one trial that measured them, they moved the average more than any single stretch of the therapy itself.

It does not mean anything about one person, and it says nothing about whether to continue, pause or restart a medication. Those are decisions for you and your doctor, and no app, Undiet included, should be part of them.

## Why most apps go blank here

Trackers are built around a program. When the program ends, the plan ends, the streak ends, and the screen that was full every morning has nothing to say.

The week after the last dose is exactly when a record matters most: appetite returning is information, not failure, and it only reads as information if it is logged next to what came before.

## Keep the diary going

In Undiet the therapy is a period on a timeline, not the timeline itself. When it ends, mark it: the medication period closes and a new one opens on the same line under whatever you are doing now.

Keep the same two taps a day, the weight and how the day felt, with the appetite slider beside them. A hungry month after the last dose looks different on a graph that also holds the quiet months before it.

If you move to a diet or a fasting window, mark that too. [Find my diet](https://undiet.app/find-my-diet) narrows the catalog to a shortlist in five taps, and [GLP-1 maintenance tracking](https://undiet.app/guides/glp1-maintenance-tracking) walks through the three phases in more detail.

## Keep the photos going

Progress photos on the way down are the ones people take. The ones from the months after are the ones they wish they had, because the mirror adapts and a photo from week 60 does not.

Keep the cadence you had, a photo a week, or go monthly if the change is slower. The ghost compare in Undiet lines up any two dates, so week 60 over week 100 is one slider away, and all of it stays on the phone.

## Keep the people

The community in Undiet shows the program and the day you are on, so a post from week 4 after GLP-1 reads as what it is, and the people replying are often in the same stretch.

Reactions are supportive only, every post is moderated and nobody can post a photo. The feed exists for the hard days, not to celebrate eating less, and credits are earned for showing up, never for kilograms.

## Where to start

The [GLP-1 page](https://undiet.app/glp1) shows the photos, the diary and the community together, with the chart of the extension data under it. The catalog entries, [GLP-1 therapy](https://undiet.app/diets/glp1-therapy), [Wegovy](https://undiet.app/diets/wegovy-semaglutide), [Ozempic](https://undiet.app/diets/ozempic-semaglutide), [Mounjaro](https://undiet.app/diets/mounjaro-tirzepatide), [Zepbound](https://undiet.app/diets/zepbound-tirzepatide) and [Saxenda](https://undiet.app/diets/saxenda-liraglutide), carry what each maker publishes and nothing more.

If you are still on the medication, [the timeline guide](https://undiet.app/guides/glp1-weight-loss-timeline) covers the numbers from the therapy itself.

## A note on advice

Undiet describes programs and records what you log. Never change, stop or restart a prescribed medication because of something you read here or saw in an app; talk to your doctor.
